Enrollment Instructions

All forms in the Enrollment Packet are required and must be typed or completed legibly.  The completed packet must be delivered to the Danvers Recreation Department, Danvers Town Hall, 2nd floor, 1 Sylvan Street by August 2, 2021.  The packet will not be accepted via email or fax and must be single side copy only.

You are required to read the Parent Handbook and sign and return the Fun Club Parent Handbook Acknowledgement Form no later than August 2nd.  If you have already submitted your Fun Club Enrollment Packet you may return your signed Parent Handbook Acknowledgement Form to Kathy Bowden, Fun Club Director, via email [email protected]

*Note: If we do not receive your complete Fun Club Enrollment Packet and the signed Parent Handbook Acknowledgement Form by August 2nd, or if your packet is illegible, this may delay or effect your child/children’s enrollment status in Fun Club for the 2021/2022 school year.
