Oil Burner/Boiler Permits
Contractor Oil Burner/Boiler Guideline
Permit Requirements:
All oil burner/boiler work shall comply with Massachusetts Comprehensive Fire Code 527 CMR 1 Chapter 11 and be documented on Department of Fire Services Form FP-056 form 1. This form can be found at the Department of Fire Services web page at: http://www.mass.gov/eopss/docs/dfs/osfm/forms/fp056.pdf
Inspection Requirements:
The Danvers Fire Department requires inspections on all work that is conducted upon oil burner/boilers and their appurtenances and the removal/installation of any aboveground/underground oil storage tanks.
For oil burner/boiler installations:
The FP-056 permit shall be submitted to Fire Headquarters with a check for $50.00 before any work is started. Upon nearing completion of the installation or modification, notify the Fire Department at 978-762-0245 and request a final inspection. Completing this inspection at this time eliminates a return visit to the site for a final inspection. The Fire Department must complete this inspection before issuing the permit for fuel storage.
Oil Tank Removal:
The FP-056 permit shall be submitted to Fire Headquarters with a check for $50.00 before any work is started. Upon nearing completion of the installation/removal or modification, notify the Fire Department at 978-762-0245 and request a final inspection. Oil tank installation and removal can be documented by (1) contractor on (1) FP-056.
